All applications must be filled out prior to arriving for your interview. All persons wishing a passport must appear in person (even minor children). Please have the following paperwork when you arrive for your appointment:

·         Completed Application
·         Original birth certificate or other proof of US citizenship (no photocopies)
·         Two identical passport photos
·         Valid picture identification
·         Check or money order made payable to Department of State (see fee schedule)
·         Cash in the amount of $25
